Extend the Validity of a Limited Passport
Requirements
1) Completed DSP-19 green application form 2) A letter that must state the request for the extension of the limited passport and the reason why ( conflicting countries or frequent travel ). 3) Release letter for adult Release letter for child 4) Government Fee - $60.00 5) Original Certified Copy of your Birth Certificate or a valid passport. 6) Copy of your Valid Drivers License 7) You do not need to make a personal appearnce at the acceptance agent. 8) Please fill out the order form and print a copy to send to us inside your Federal Express Priority Overnight.
