Name Change
Note: If your passport is going to expire within 1 year you should follow the renewal instructions.
Requirements
1) DSP-19 green amendment form 2) Your passport which must still be valid. 3) Proof of your name change, such as: (originals only) a) marriage license or b) divorce decree or c) court order or 5) Government Fee - $60.00 6) If you need a passport within 10 business days, then you will also need proof of departure, which will be ONE of the following: a) copy of airline tickets or b) travel itinerary or company letter of expedite(click for sample) 7) Please fill out the order form and print a copy to send to us inside your Federal Express Priority Overnight. Fax order form with credit card information for charge of $99 plus fed ex charges. Once you have the above in order please call us at to go over your information.
