New Passport/Child Passport
Follow these instructions if:

    You are applying for a new passport/child passport for the first time.

    You are applying for a minor (under16).

    Your passport was issued more than 15 years ago.

    Your passport was issued before you turned 16.

    In order for us to process your new passport/child passport, you must first go to a
    local acceptance location which will either be a special post office or county clerk's office. Make sure you take sufficient identification, such as a driver's license, with you. You will need to show the items listed below, which will then be put into a government sealed envelope. Make sure you get the envelope back from the clerk, and do not open it.

Requirements

1) DSP-11 brown passport application

2) Two Passport Photographs taken by a passport photographer.

3) Original Certified Birth Certificate, or original naturalization certificate or the most recent passport. If applying for a Child passport and you are providing a previous passport for your child, you must include a certified birth certificate in addition to the previous passport to prove who the child's parents are.

4) 1 Check/money order made payable to Department of State for $135 for adults and $120 for minors and $30.00 for the post office/county clerks fee. No temporary checks will be accepted by the passport office. You can use a money order in place of a temporary check.

5) Valid Picture Driver's License issued more than 6 months ago (no paper licenses).If your Driver's License has been issued or renewed in the last 6 months, you will need to provide at least 3 personal documents.

6) You will need to furnish one of the following for proof of departure:

     a) copy of your airline tickets or

     b) travel itinerary or

     c) company letter of expedite (sample)

Release letter for ADULT ( must print out )
    Release letter for CHILD ( must print out )

7a ) Both parents must appear with the child at the acceptance agent when obtaining the sealed envelope. If both parents cannot be present then a Written Statement of Consent must be provided to the acceptant agent.(The Written Statement of Consent MUST be notarized.).

In order for us to process your new passport/child passport, you must go to a local acceptance location (Every person must appear in person as of 6-17-04) which will either be a special post office or county clerk's office.

Make sure you take sufficient identification, such as a driver's license, with you. You will need to show the items listed above, which will then be put into a government sealed envelope. Make sure you get the envelope back from the clerk, and do not open it. Once you have your sealed envelope you will need to Federal Express the sealed envelope and the Passoprt Order Form on line # 8 below with the payment for our services to the address on the top left of the Order form.

8) Please fill out the Passport Order Form and print a copy to send to us inside your Federal Express Priority Overnight.

Before you go to your local acceptance location (Either a special post office or county clerk's office) please call us at (888) 629-0087   so we can go over your information with you.We will make sure everything is in order. After you appear at your local acceptance agent you will then Federal Express your sealed envelope, order form and payment to us at:
